Hospice Medical Director
is an expert in Hospice and Palliative care. An active team member, who guides the team, makes certain that the right plan is in place, and ensures that the team is providing the highest level of pain management.
Patient Primary Care Physician is responsible for their patient’s medical care and continues to be actively involved.
Hospice Nurses under the direction of the physician, manage pain and other symptoms, such as nausea, shortness of breath, fatigue, loss of appetite, constipation and sleep disturbance. Nurses are available 24-hours-a-day / seven days a week to respond to patient and family needs.
Social Workers relieve the emotional stress on families who are facing critical life issues, while coordinating and ensuring that community resources are easily accessible.
Home Health Aides provide personal care for Hospice patients ensuring that quality of life and dignity are preserved.
