| Hospice of Siouxland has partnered with nursing facilities in the Siouxland area to enhance end of life care regardless of their residence. Services are available for persons currently residing in a nursing facility or when a hospice patient can no longer be cared for in their home setting.
Advantages to the Patient and Family
- The focus is on the patient and family as a unit of care.
- Hospice team members of nurses, social workers, home health aides, and volunteers are available to visit the patient and family and with the nursing home personnel work to enhance comfort and quality of life.
- Often families experience a decrease in the cost of care. In most instances, medications, medical supplies, equipment, and treatments related to the life limiting illness are covered by hospice.
- Through the Hospice bereavement program, the family of the nursing home patient continues to receive support for up to 13 months following their loved one's death.
